REGISTRATION & TUITION

� Tuition for the MVP Institute is $350 per person. This fee includes a full copy of the MVP curriculum, certificate of completion, and continental breakfast each day. A limited number of scholarships are available. Participants are eligible to receive 21 PDP�s from the Massachusetts Department of Education.

� To enroll in the MVP Institute, please complete the registration form to the right and mail with check, money order or purchase order to the return address on the front of this brochure. Payment is due prior to or on the first day of the Institute. Checks should be made payable to Sport in Society. Once your registration is received, you will be sent additional information
with confirmation. Space is limited, so please act now!

� The MVP Institutes will take place from 9 AM�5 PM, January 22 -24, 2007 and August 6 - 8, 2007. Successful completion of the MVP Institute is contingent upon full attendance and participation at one of the MVP Institutes.
